New Clients

Documents
During your initial call it will be helpful to gather the following documents:
1. Proof of Address
Acceptable proof: utility bill with your name; or rental/lease agreement with your name; or driver’s license. If you are unable to obtain the above documents (e.g., recently moved, living with family or friends), please bring someone who knows your living situation with you to enroll.
2. Government Issued Photo ID
If no photo ID is available, we can assist in applying for a photo ID.
3. Proof of Income
Eligible proof: two recent pay stubs; income tax return from the most current year; proof of child support; social security benefits; disability benefits.
